  • Identity Data: We may collect first name, last name, username or similar identifier, marital status, title, date of birth and gender.
  • Contact Data: We may collect includes email address and telephone numbers.
  • Identity Documents: We may collect identity documentation such as copies of driver’s licenses.
  • Security Credentials Data: We may collect user IDs, passwords, password hints, and similar security information required for authentication and access to One Degree accounts.
  • Technical Data: We may collect your internet protocol (IP) address, your login data, browser type and version, time zone setting and location, browser plug-in types and versions, operating system and platform, and other technology on the devices you use to access this website.
  • Usage Data: We may collect information about how you use our website and services (Note: we do not currently collect this type of data)
  • Marketing and Communications Data: We may collect your preferences in receiving marketing from us and our third parties and your communication preferences. (Note: we do not currently collect this type of data)

We may collect this information: (a) directly from you when you provide it to us; or (b) automatically as you navigate through the Website. Information collected automatically may include usage details, IP addresses, and information collected through cookies, web beacons, and other tracking technologies. 

Some of this information is required for us to provide this service to you, and some of this information is optional.  

We may also receive information about you from suppliers, those businesses that provide us services (such as service providers, contractors, and consultants), third parties, or other online or offline sources. We may combine such information with the information you have provided to us in order to provide and improve our services.

How we share your data

We will only share your personal information as follows, and, when applicable only with the appropriate contractual obligations in place:

  • Sharing between One Degree platforms: We may share information between our One Degree products. 
  • Sharing with service providers and partners: We engage service providers and partners to manage or support certain aspects of our business operations on our behalf. Our service providers and partners are required by contract to safeguard any personal information they receive from us and are prohibited from using the personal data for any purpose other than to perform the services as instructed by One Degree. We do not share personal data (phone numbers) and consent with third parties / affiliates or partners. No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
  • Business transactions: Circumstances may arise where, whether for strategic or other business reasons, One Degree decides to sell, buy, merge or otherwise reorganize businesses. In such transactions, we may disclose or transfer your personal data to prospective or actual purchasers or receive personal data from sellers. Our practice is to seek appropriate protection for your personal data in these types of transactions. 
  • Compliance with law: Where we are obligated to do so, we may also disclose your personal information to comply with any court order, law, or legal process, including to respond to any government or regulatory request.
  • NorthEast Valley Health Corporation: If you are a NEVHC patient or member and you choose to complete a health assessment on this site, your assessment data will be shared with NEVHC so they can better serve your health needs. 

We may also disclose aggregated information about our users. We take appropriate steps to ensure that it is not likely reasonable for someone to re-identify aggregated data. 


How we store and retain your data

We securely store and retain personal information for as long as necessary to provide you with the services that you have requested. We may also retain and use this information for a longer period for the following reasons, as permitted or required by law:  

  • to comply with our legal, tax or regulatory obligations (e.g., audit and accounting requirements);  
  • to handle disputes; and 
  • to exercise or defend claims and enforce our agreements.  

We ensure that personal information we dispose of, is de-identified or destroyed in a secure manner.  

We may retain de-identified and other non-personal data indefinitely. We de-identify such information such that it is not likely reasonable for someone to re-identify the data.


How we protect your data

We are committed to ensuring that your data is secure. To prevent unauthorized access or disclosure, we have put in place reasonable physical and electronic controls to secure the data we collect.

We follow generally accepted industry standards to protect information submitted to us, both during transmission and once we receive it. However, no method of transmission over the Internet, or method of electronic storage is 100% secure. Therefore, while we strive to protect your personal information in our possession, we cannot guarantee its absolute security. Children’s privacy

We do not knowingly collect, maintain, or use personal information from children under 13 years of age without appropriate parent or guardian consent, and no part of our Site is directed to children.

If you learn that a child has provided us with personal information in violation of this Privacy Policy, then you may alert us at the address found in the ‘How to contact us’ section and we will promptly delete any such information that we have collected.

We keep our privacy policy under regular review, and it is our policy to post any changes we make to this policy on this page.
